Description Due to business growth and our ability to coordinate services for more adults with physical disabilities, The Coordinating Center is seeking several qualified candidates to join their Service Coordination Team. The Service Coordinator is a center-based position providing coordination of services in conjunction with the Clinical Care Coordinators working on the Living at Home interdisciplinary care management team. The Service Coordinator supports the mission of The Coordinating Center by providing person and family-centered coordination services and adhering to the parameters of the Living at Home contract. Service Coordinators are responsible for complying with specific contract requirements, integrating technology to maintain database integrity, report writing and maintenance of documentation. The Service Coordinators work interdependently with The Center's Living at Home team to maximize quality outcomes for individuals to live in the community.

Essential Responsibilities:

The Service Coordinator in conjunction with the Clinical Care Coordinator is responsible for providing care coordination for adults with physical disabilities and is the primary point of contact with clients by performing on-site visits, information gathering, monitoring of plans of service, and completing telephonic care coordination with clients:

• Secures information such as medical, psychological, and social factors contributing to client's situation, and evaluates these and client's capacities to assist in the development of plans of service for coordination services.

• Promotes positive health behaviors to support optimal health and well being.

• Provides consistent telephone contact with client, family members and caregivers to assist with the coordination of services per The Center’s protocol adhering to contract requirements by monitoring the existing plans of service.

• Collects information to assist the development of plans of service provided to clients.

• Accesses and records client and community resource information and care coordination activity.

• Manages multiple priorities and tasks with flexibility to enhance service coordination services
Requirements Qualifications:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or abilities required:

• Bachelor's degree (B. A.) from four-year college or university in health/human services or related field.

• Two years related experience and/or training and working knowledge of housing, social service and individual support services; Medicaid services or Home and Community Based Waivers is desirable.

• Ability to communicate effectively with individuals with varied cognitive abilities to establish professional relationships. Ability to read and interpret documents such as employment paperwork, assessment reports or medical records, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or co-workers of the organization.

• Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

• Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

• Ability to travel throughout the state of Maryland estimated to average about 50% of work schedule
