Description: Full Time Adult, Child and Intensive Case Managers

Our Case Manager’s provide case management services that includes the following elements: assisting clients in obtaining access to financial and insurance benefits, medical, educational, social and functional abilities, needs and other services as needed.  All of these elements require working with our client’s and their extended support network’s to develop, implement and monitor treatment plans and goals.   

• Maintain an active caseload of clients
• Completes program intake and assessment with clients
• Accompanies clients to and links clients with community resources as needed
• Supports client and families, where appropriate, on relevant clinical issues
• Bi-Monthly home visits to all clients on caseload
• Monthly client contact with each client on caseload
• Advocates for client, such as assisting client in completing applications for services and discussing client’s needs with the provider
• Coordinated care with psychiatrists, therapists, medication clinics as well as external services such as hospitals, social service agencies and household agencies
• Monitors treatment process to ensure needed services are provided on timely basis

Full Time Adult, Child and Intensive Case Managers

Requirements : 
 

Graduation from an accredited university with a minimum of a Bachelor’s Degree in Psychology, Social Work, Marriage and Family Therapy, Mental Health Counseling or Criminal Justice with at least 1 year experience working with the specific population they are applying to work with (adults with serious mental illness) and:

• Valid FL Drivers License

• Must have knowledge and must comply with state and federal statutes, rules and policies that affect the target population

• Must have knowledge of available resources for the specific population that they are working with

• Must have knowledge of the principles and practices of the discipline

• Demonstrates proficiency in verbal and written skills
• Ability to work with diverse groups and individuals
• Must successfully complete all criminal background screening
