Description Presbyterian Children's Services currently has an opening for a Family Foster Care Case Manager. The person chosen for this position will work with  children and families ensuring safety, services, and permanency for children at risk of abuse and/or neglect.

Primary Duties:

     -Develops individualized care plans.
     -Coordinates individualized services and providers working in the home, school, and
      community environment.
     -Facilitates Family Support Team meetings.
     -Records and documents all support related activities and services.
Requirements Master's degree in social work, psychology, counseling or a closely related discipline and 2 years experience OR bachelor's degree with 3 years experience in providing wraparound services, community-based services, or case management services.

Applicants should be able to work irregular hours, be able to handle crisis in a calm and professional manner, and have good physical and emotional stamina. Must also be able to pass an extensive background screen.
