Family/Community Specialist-PJF

SALARY: $2,514.00 /Month

PRIMARY RESPONSIBILITY:

To provide resource coordination, case management, promote and maintain the health and development of each child and their families in Adams County Head Start. Develop and maintain relationships with agencies that provide services in the community.

 

EXAMPLES OF IMPORTANT AND ESSENTIAL DUTIES

·          Responsible for recruitment, and enrollment of Head Start families

·          Provide orientation to families regarding the Head Start program when they transition into the program

·          Build rapport with families of diverse cultural backgrounds

·          Conduct home visits as needed

·          Develop strength based Family Partnership Agreements with all Head Start families

·          Responsible for connecting families with resources to achieve their desired goals and monitor the referrals

·          Responsible for providing case management to families to support them in reaching or maintaining self-sufficiency

·          Provide crisis intervention to families, ensuring the safety of all involved, identifying options/referrals and supporting families in making decisions

·          Responsible for maintaining 85% attendance in head Start classroom, through communication, support and follow up with Head Start families

·          Assist parents in planning, facilitating and setting up for evening Parent Committee meetings

·          Prepare monthly reports for supervisor regarding family partnership agreements, attendance, and referrals

·          Participate in agency self-assessment and other efforts to improve the quality of program services

·          Responsible for communicating pertinent information to appropriate ACHS staff concerning the child or family to coordinate comprehensive case management

·          Develop and coordinate kindergarten transition plans for Head Start children and families

·          Maintain professional boundaries and confidentiality.

·          Participate in monthly staff development trainings

·          Perform other related duties and responsibilities as required.

 

REQUIREMENTS

Experience: One year experience in social work, case management or related field

Training - A Bachelors degree with major course work in social work or a related field.

License or Certificate: Valid Colorado Drivers License and Proof of vehicle insurance

Ability to pass CBI & FBI background check and Central Registry check. Must complete physical examination and TB test upon hire.
