Location:Providence Tarzana Med Ctr
Providence Tarzana Med Ctr.,located in Tarzana, Ca. is a 245 bed hospital
providing premiere healthcare services in the San Fernando Valley.
Summary: Reports to the Dir., of Case Mgmt. The case mgr ensures timely
discharge through early intervention and identification of post-hospital care
needs. Completes timely reviews utilizes Interqual criteria for all reviews,
participates in rounding and discharge planning as a laison to hospital staff,
patients families and physicians.
Position Requires:
RN license & BLS card BSN preferred.
Minimum Experience:
Ability to communicate effectively in English both verbally and in writing. 3
yrs nursing experience preferred. 3 yrs case mgmt experience in acute care
preferred.
Preferred Skills:
Knowledge of Medicare, MediCal regulation and completion of TARS. Knowledge of
InterQual criteria.
