CHC currently seeks an experienced primary care provider as a Family Practice Physician or Family Nurse Practitioner.

 

As a primary care provider, you will:

 

 

 

 

  • Provide a full scope of medical services largely geared to underserved populations
  • Receive strong clinical support services and work in an innovative practice environment utilizing Advanced Access scheduling
  • Be part of a multi-disciplinary practice that offers a blend of individual and population based care
  • Be on call no more than one in four nights and weekends

 

Loan Repayment Opportunities:

 

 

If you’re a health care professional who is hired by Community Health Center, you may be eligible to receive up to $500,000 tax-free through the federal Loan Repayment program. The federal Loan Repayment program is administered through the National Health Service Corps (NHSC) Loan Repayment Program. The National Health Service Corps Loan Repayment Program provides $50,000 (or the outstanding balance of qualifying student loans if it is less than $50,000) to primary care medical, dental and mental health clinicians in exchange for two years of service at an approved site.

Job Requirements 

 

  Physician Qualifications:

 

  • MD/DO from approved medical/osteopathy school and graduation from accredited residency program in family practice
  • Must be board-certified within 24 months of commencing employment at CHC and hold license or be eligible for license in State of CT
  • Bi-lingual skills English/Spanish preferred

 

APRN Qualifications:

 

 

 

 

 

  • Graduate of an accredited Master of Science in Nursing program with a specialty as a Family Nurse Practitioner
  • ANA certification as a Family Nurse Practitioner
  • State of Connecticut APRN and RN licensure (within 12 months) required.
  • Bi-Lingual Skills English/Spanish preferred
