 Description CLINICAL LIAISON - CASE MANAGER NEEDED (Marketing/Sales background preferred)!




Responsibilities for Clinical Liaison:
Clinical Liaisons with interact with discharge planners, case managers, clients and families at designated facilities to assist the initiation of home healthcare services and the discharge planning process.

Using your knowledge of eligibility criteria/regulations and clinical experience to help identify potential clients, you?ll facilitate communication and develop relationships with referral sources, physicians and other healthcare professionals to increase referrals.

Though focused primarily on reviewing medical documentation, working with physicians to make clinical judgments, and coordinating care with the service office, you?ll also participate in community and marketing activities as needed.

We are in need of a self-motivated RN who can manage and facilitate the safe and timely transfer of patients from an acute-care setting into a skilled nursing or rehab setting.

This position will also assist existing referral sources in identifying potential clients who could benefit from our services.

The successful candidate will have a history of building solid client relationships, maintaining excellent customer service, and helping identify potential home care clients.

Qualifications needed for Clinical Liaison:
•Must have a current license as a Registered Nurse, LVN or Respiratory Therapist.
•Two (2) years experience in clinical practice?a history of discharge planning experience and/or patient advocacy is preferred.
•Demonstrated customer service ability
•Excellent organizational, interpersonal, telephone, documentation, and communication skills
•Ability to travel to local facilities and physician offices as needed.
