Lake View Care Center

has been a trusted long term care and rehab center for many years, fully accredited and licensed by the state of FL.  We are a 120 bed skilled nursing facility committed to providing excellent rehabilitation services to our residents.  Whether the need is short-term rehab, skilled nursing care, long term or hospice care.

 

 Requirements

 

  • The SLP will provide evaluation and therapeutic services as needed by our patients.
  • He/She will plan, develop, organize, implement and evaluate direct therapy in accordance with current rules regulations and guidelines that govern long-term care facilities.
  • Participate in discharge planning, implementation and development of residence care plans, residence assessments, etc.
  • Certified licensing in FL State as a Speech Pathologist.
  • SNF experience preferred.
