Intensive Case Manager --- Bilingual [ Spanish / English ]

Job Content: It is the responsibility of the Intensive Case Manager to provide community-based services to individuals identified as eligible for mental health services, including accessing and monitoring of resources that help the client live in the community in a stable, safe manner. The following elements are considered essential job functions, unless otherwise noted.

Carrying a case load of up to 30

Performing case management duties for consumers involved in Mental Health Services

Attend treatment team meetings weekly
-
Holding a close working relationship with mental health providers

Attending weekly supervision and staff meetings
-
Need to be availalbe for an on-call rotation, 24/7
Minimum Education: Bachelors degree in a behavioral health discipline or equivalent in experience, training and college credits.
Minimum Experience: At least one year of experience in the behavioral health field.
