Responsibilities to include but not limited to:
To provide medically prescribed evaluation and treatment of individuals following accepted practice guidelines, in a home health care setting.
Render treatment to relieve pain, restore or develop function, and maintain maximum performance according to accepted physical therapy practices for the purpose of improved patient outcomes.
Direct and aid the patient in active and passive exercises, muscle re-education and training of activities of daily living.
Instruct family and caregivers on the total physical therapy program and the procedures that will be followed for maximum benefit.
Comply with state and federal supervision requirements.
Coordinate every fifth patient visit with the supervising physical therapist.
Education
Associate of Science degree in Physical Therapy from a school approved by the American Physical Therapy Association is required.
Experience
A minimum of one (1) year home health experience is preferred.
Certificates & Licenses
Current license to practice a Licensed Physical Therapy Assistant in the appropriate state, current driver's license, automobile liability insurance, and CPR certification.
Other Requirements
Demonstrates excellent verbal and written communication skills. Must effectively communicate with patients, families, and others and convey sense of caring, concern and dignity for patients.
Accurately interprets the patient's physical therapy home care needs. Makes prompt and accurate physical therapy judgments while following the supervising physical therapist's treatment plan.
