JOB SUMMARY
The Clinical Program Coordinator is responsible for programmatic oversight, planning and coordination of patient care throughout the continuum of services from initial referral to follow-up care. May also serve as the primary clinical coordinator for the interdisciplinary team which may include: physicians, nurses, social worker, dietitians, pharmacists, financial analysts, case managers, and support staff as applicable.
Assists the Service Line Manager through the provision of onsite leadership and supervision of nursing and clinical support staff to ensure the prompt delivery of patient care and efficient flow of clinical operations. Participates in the development of guidelines, protocols, pathways and other performance improvement activities. Develops, coordinates, implements, and evaluates educational programs/projects for patients, personnel, and the community. Collects/documents data to meet clinical, financial, and regulatory requirements. May participate in research activities and represent the Medical Center at regional and national meetings/conferences.
Provides nursing care consistent with Nursing Standards of Care/Practice, the Plan for Provision of Nursing Care, The Plan for Provision of Care to Patients, CSMC Code of Conduct, the Performance Improvement Plan, the Patient Education Plan and Nursing Strategic Plan. He/she maintains competency skills including the ability to provide care to special patient populations and patient with diverse cultural backgrounds. He/she can identify the ability of the patients to follow directions/instructions and give consent. He/she can identify safety concerns and institute measures to provide a safe environment; and provide appropriate assessment, intervention and evaluation of patients with consideration of developmental milestones in the patient population.
EDUCATIONAL REQUIREMENTS:
- Bachelor of Science in Nursing, preferred.
- Certification required in accordance with organizational requirements.
License/Certification/Registration Requirements:
Current California Registered Nurse License
EXPERIENCE:
- Relevant clinical experience for population served.
- Excellent organizational skills and ability to effectively coordinate all aspects of patient care.
- Excellent judgment and application of critical thinking skills in all aspects of leadership and supervision.
- Ability to communicate effectively, both written and verbal communication, with persons of various cultural and educational backgrounds.
- Available 24 hours/day if scheduled for on-call activities. (varies by program)
- Ability to be an active, participatory member of the interdisciplinary team.
- Full command (verbal and written) of the English language.
- Computer proficiency.
- Prior supervisory or charge nurse experience.
- Demonstrated knowledge of case management.
- Experience in data management.
