Job Description

Under general supervision, renders professional nursing care to adults and geriatric patients of other ages when indicated through the use of the nursing process and through the promotion and maintenance and teaching of health. Notifies the charge nurse of any problems on the unit. Follows the philosophy, policies and goals of the hospital, nursing services department and Rehab Care.  To care for the patients of the Transitional Care Unit by providing both direct and indirect care based on the patient needs to include teaching, discharge planning and care of medical conditions as directed by the patient’s physician. To be able to provide for non-emergent and urgent care as the needs arise. To provide for the well being for the patient entrusted to the care of Transitional Care Unit inclusive of the physical, emotional and psycho-social issues that arise during their stay in the unit

Job Requirements

Education:


  • Graduate of an accredited school of nursing

Certifications / Licenses:


  • Current state Nurse license
  • Maintain Basic Life Support (BLS/CPR) Certification
  • Maintain Advanced Cardiac Life Support ( ACLS) Certification
