Group Fitness Instructor - Independent Contractor

Schedule:  As Needed

Department:  Lifetime Fitness Center

Location:  Story City Facilities

Agrees to schedule and provide fitness class instruction services as agreed upon to clients of MGMC’s Lifetime Fitness Center.

Performs any and all tasks and duties associated with the Scope of Work. The Independent Contractor shall not be entitled to engage in any activities which are not expressly set forth by this Agreement.

The books and records related to the Scope of Work shall be maintained by the Independent Contractor at the Story City Lifetime Fitness Center and shall be the property of MGMC.

Comply with Medical Center orientation, health, education and competency assessment requirements as outlined by Human Resources policy.

Responsible to the MGMC Rehab & Wellness Director regarding the quality of service and conformance with MGMC's policies and procedures. Upon signing and returning this agreement to MGMC, the Independent Contractor agrees to follow and complete all requirements according to Human Resources (HR) policies. The Human Resource Department shall inform the contractor of all requirements and shall provide the documents and explanation necessary for completion of the HR process.

Any instructor assigned to MGMC under the terms of this Agreement shall possess appropriate training experience as a Group Fitness Instructor.
