The following statements are intended to describe the general nature and level of work being performed by individuals assigned to this position. They are not intended to be an exhaustive list of all duties, responsibilities and skills required of personnel so classified.

• Confer with physician to develop the initial plan of treatment based on physician’s orders and initial patient assessment.
• Provide hands-on care within the clients place of residence, case management and evaluation of the care plan, and education of the patient and family members responsible for patient care. Revise plan in consultation with physician based on ongoing assessments.
• Conduct supervisory visits of LPN/LVN and CNA/HHA as needed.
• Responsible for chart reviews to ensure accurate and timely documentation of services and compliance with regulations. Ensures patient records are maintained accurately and in a timely manner.
• Has the autonomy to make individualized patient-centered decisions for optimal clinical care.
• Works as a part of interdisciplinary team to provide the best outcomes for our patients.

• Must have the ability to function autonomously and possess effective time management skills.
• Self-directed and able to work with little supervision.
• Appropriate clinical judgment and communication skills. Evidence of leadership abilities.
• Ability to establish and maintain a courteous and cooperative (service-oriented) working relationship with patients, families and co-workers.
• Must be able to perform the physical requirements of the job.
• Absolute love of patient care!

Experience Required

• Associate or bachelor degree in nursing.
• Current and unrestricted RN license in state of agency operation.
• Current CPR certification.
• One year RN experience to include med surg, ICU, ER, telemetry or acute care.
• Experience working with an interdisciplinary team.
• Prior home health care experience a plus. OASIS charting experience a plus.
• Must have valid state driver’s license, reliable transportation and auto insurance.
