Habit OPCO provides rehabilitation treatment services on an outpatient basis for patients with drug addictions, enabling them to function normally in society. Our highly-trained professionals, many of whom are Masters-prepared, provide individual, group and family counseling focused on addiction, recovery, and related personal, family, and vocational issues.

As a Care Coordinator, you will become an integral part of Habit OPCO’s multidisciplinary treatment team, helping patients in accessing medical, clinical, vocational, and other rehabilitative services as indicated in the biopsychosocial assessment.

As a Care Coordinator you will act as liasion between patients and collateral agencies providing Spoke service. The Care Coordinator assists patients in accessing medical, clinical, vocational, and other rehabilitative services.
Your specific duties as a Care Coordiantor are, but not limited to:
- Facilitates referrals to Spoke agencies based on assessments of need.
- Documents all contact with patients and provider using appropriate service in the eectronic health record
- Obtains and provides intake referral informtationto patients in a timely manner
- Ensures that a minimum of one health home function and one face-to-face clinical contact is provided for each Hub patient on a monthly basis.
- Perform monthly audits of coordinating activities to supervisor and MDT.
