Description We are a private duty home care company that has been providing exceptional personal care and support services for over 20 years.  LifeStyle Options is nurse owned and operated and is a member of the Better Business Bureau, National Private Duty Assocation and was accredited with JCAHO 3 times during the history of our business.

Position 1
Nurse Manager (RN or LPN)
Work in a beautiful retirement community in Niles or NW Chicago.  Full-time position.  Duties for this position include, but are not limited to:
- Supervision of home care private duty clients and convenience services
- Supervision and scheduling of our your Caregiver team. 
- Opening assessment visits for new cases
- Wellness services and marketing
- Medication Management and Administration
- other duties as assigned.


Position 2
Nurse Supervisor:  (RN only)
Work a flexible schedule for 20 hours per week, with the opportunity to work some days from home.  Duties for this position include, but are not limited to:
- Supervision of home care private duty cases
- Supervision of our CNAs and Nurses who work these cases
- Opening assessment visits for new cases
- Geriatric Care Management
- Medication Management and Administration
- Assist Operations with written procedures
- other duties as assigned.


Requirements - Current nurse licensed
- Current CPR certified
- Health Exam
- TB test result
- Valid Drivers license

- Experience in home care or Geriatric Care preferred
