Welcome to Senior Home Care. As one of the nation's leaders in home health care, Senior Home Care has been taking care of patients in the comfort of their homes for 20 years. Who We Are


Founded in 1994 and headquartered in Clearwater, Fla., Senior Home Care is a privately held company operating nearly 50 branch locations with 1,800 employees through its Senior Home Care offices in Florida and Synergy Home Care offices in Louisiana. Our skilled clinicians are dedicated to caring for more than 50,000 patients each year.

Senior Home Care has 30 office locations and its corporate office in Florida. In 2003, Senior Home Care acquired Southeast Florida' s Med Tech Services, a well known home health provider in the region for over 20 years. With staff based in five branches, the company services patients throughout Indian River County to Miami-Dade County and all areas in between. In December 2012, Med Tech Services changed its name to Senior Home Care to better align its identity with the company's other locations statewide. 


Synergy Home Care Branches Throughout Louisiana


In 2007, Senior Home Care acquired Synergy Home Care, a leading provider of home nursing services for the Medicare population of Louisiana, with 17 locations across the state. Synergy Home Care was founded over 15 years ago and expanded through acquisition and the opening of new locations. At the time of the Senior Home Care acquisition, Synergy employed over 600 health care professionals who enthusiastically live the mission of providing unsurpassed quality patient care.

Our Credentials

Senior Home Care and Synergy Home Care are accredited with The Accreditation Commission for Health Care, Inc. (ACHC) as a demonstration of our commitment to providing excellence in home health care services. In 2012, Senior Home Care also became the first large provider of its kind to earn accreditation for Behavioral Health Home Care (BHHC) from the ACHC (achc.org). BHHC integrates behavioral health interventions into home care services for patients whose mental illness, substance abuse, intellectual/developmental disabilities make it difficult, or otherwise prevent them from receiving care outside of their homes. ACHC is ISO 9001:2008 certified and has Center for Medicare and Medicaid Services (CMS) Deeming Authority for Home Health, Hospice and DMEPOS (Durable Medical Equipment Providers).


Senior Home Care and Synergy Home Care were proud Premier Participants in the 2010-2011 Home Health Quality Improvement (HHQI) National Campaign. HHQI is a grassroots movement designed to unite home health stakeholders in: reducing avoidable hospitalizations, improving medication, and preventing falls in the home and in cross-setting care.


In January, 2013, Senior Home Care was the first home health provider to join the Florida Health Information Exchange (HIE). Florida HIE provides health entities with a timely, secure and authorized exchange of patient health information for treatment purposes. As part of the HIE, Senior Home Care is committed to improving the quality of health care through better outcomes, lower costs with excellent patient satisfaction.


In 2012, Senior Home Care was named HomeCare Elite™ for the seventh year in a row. HomeCare Elite is a compilation of the top home health agencies in the United States and is managed by OCS HomeCare, the leading provider of home care information, and DecisionHealth, the industry's leading source for news (publisher of Home Health Line). HomeCare Elite is compiled from publicly available data from Home Health Compare and The Centers for Medicare and Medicaid Services (CMS) Cost Reports. The three categories used to calculate the results are quality of care, quality improvement and financial performance.
 

In January, 2013, Synergy Home Care was among the first home health care providers to join the Louisiana Health Information Exchange (LaHIE). The LaHIE is the mechanism that allows for the secure exchange of health information among authorized providers and organizations across Louisiana’s health care system to help improve patient safety, quality of care and health outcomes.


Medicare

Medicare is government-sponsored health care coverage for seniors and the disabled. It offers different ways for people to choose their coverage so individuals can pick a plan that fits their needs best, based on cost, benefits, doctor choice, convenience and quality.

  • Medicare Part A covers hospital stays, emergency care, skilled nursing facilities, hospice and home health care, if certain conditions are met.
  • Medicare Part B covers doctor's visits and some routine preventive care.
  • Medicare Part C, also known as a Medicare Advantage Plan, is managed by private insurance companies approved by Medicare and combines Part A and B coverage.
  • Medicare Part D helps cover the cost of prescription drugs.

Patients of Senior Home Care can use the benefits of Medicare to cover the cost of home health care services. If you have original Medicare, you may receive services from Senior Home Care at no cost to you. Some patients may choose to use a replacement plan or Medicare Advantage plan, which may involve a deductible or co-pay.

Original Medicare pays 100% for home health services when the following conditions are met:

  • Medicare eligible
  • Homebound
  • Plan of Care (485)
  • Care is intermittent and part time
  • Doctor’s orders
  • Documentation of all services
  • Care is reasonable and necessary

Our talented therapists, nurses and clinicians utilize a multi-disciplinary approach to care, ensuring everyone involved in the treatment process is in regular communication.
