CHANNELING
At Home and Independent
Provides an alternative to nursing home placement through the coordination of community services and family support. The Channeling program seeks to assist frail elders to live independent, active lives in their own homes.

DOUGLAS GARDENS COMMUNITY CARE
A Lead Agency for Miami-Dade County
An alternative to nursing home care that provides high quality in-home services through individualized case management to assist eligible frail elders remain as independent as possible. The program is sponsored by the state of Florida, Department of Elder Affairs through the Alliance for Aging.

MAP (MEDICAID ALZHEIMER’S PROJECT)
Quality of Life, Always
A special Medicaid program developed to meet the extraordinary needs of individuals with Alzheimer’s Disease and their families.

PROJECT INDEPENDENCE AT HOME
Seniors Living Life to the Fullest
A special Medicaid program that provides people 65 years or older with the services they need to remain as independent as possible in the community.
