As the oldest provider of home health care in southwest Michigan, we have decades of experience. For 50 years and counting, we’ve been working to provide innovative, compassionate solutions for a wide variety of people. In addition to being fully certified by Medicare, Medicaid and Blue Cross Blue Shield of Michigan, we are also accredited by the Community Health Accreditation Program (CHAP), so you can always be assured of the highest standards of care.
Borgess Visiting Nurses Hospice
is the oldest provider of home health care in southwest Michigan, serving the counties of Allegan, Barry, Berrien, Calhoun, Cass, Kalamazoo, St. Joseph and Van Buren.
Our agency was founded specifically to provide care in the home through home health care, hospice care and related services.

Our Mission
The Mission of Borgess VNA Home Care is to provide individuals and families with compassionate, quality, innovative and cost effective home care, hospice and related health services.
Services
Borgess Visiting Nurses
provides free in-home Health Care Assessments to individuals who are likely to meet criteria for home health care services including:
1Currently receiving Medicare/Medicaid benefits
2Homebound, which is defined as:
Leaving the home would require taxing effort.
The person has a condition due to an illness or injury that restricts his/her ability to leave home except with the aid of supportive devices such as crutches, canes, wheelchairs, walkers, special transportation or the assistance of another person.
A patient has a condition such that leaving his/her place of residence is medically contraindicated.
Absences from the home are infrequent, for periods of short duration for the purpose of receiving medical treatment, attendance at day care centers or religious service.
The in-home assessment is performed by a home care registered nurse, takes one to two hours and includes:
Blood pressure check
Heart Rate
Weight
Pain Assessment
Medication
Physical Assessment, including ears; eyes, nose, mouth; mental, neurological, muscular, cardiovascular and respiratory status
Review General Health Screening Review to document past immunizations, mammograms, cervical and testicular cancer screenings
Diet & Nutrition Assessment
Assess Self-Care Abilities
Summary of home care needs, including recommendations for community services
In addition to those listed above, persons who meet Medicare/Medicaid or Blue Cross and Blue Shield criteria for home health care must have the following assessments performed to see if they qualify for future home health care benefits:
Comprehensive Safety Assessment
Comprehensive Assessment of Activities of Daily Living, including grooming, feeding, etc.
Health Risk Factor Assessment
Caregiver Assessment
Teaching/Learning Assessment Related to Self-Care
If the nurse finds that you meet the criteria for home health care services, the nurse will begin the process for admission into home health care services.
Borgess Visiting Nurses will also contact your physician to obtain an order for future home health care visits. You may have a family member or friend present. If you do not meet the Medicare criteria for home health care, Borgess Visiting Nurses will refer you to other services based on need, including private duty home health care through Borgess VNA Home Care, senior services, meals on wheels, etc.
